Long-term follow-up care survey for young cancer patients, survivors and healthcare providers

Did you know that most childhood, adolescent, and young adult (CAYA) cancer survivors do not receive appropriate long-term follow-up (LTFU) care which can significantly impact their quality of life? Please help us improve survivorship care across Europe!

The main aim of this survey is to gain knowledge about current existing LTFU care for childhood, adolescent, and young adult cancer survivors within Europe and identify best practices, facilitators, and barriers in those areas.

To get the most comprehensive picture possible, your input and experience are essential to us.

By gathering data from both CAYA cancer survivors and healthcare professionals, these results can identify key areas for improvement in survivorship care and ultimately improve the overall well-being of survivors and their quality of life.

Are you a childhood, adolescent, or young adult cancer survivor? We want to hear from you! Your experience with long-term follow-up care is valuable to help us improve survivorship care within Europe. Please fill out the ‘survivor’ survey. We also encourage parents/caregivers to fill out this survey in order to share their reflections upon LTFU care.

Are you a healthcare professional working in Long-Term Follow-Up (LTFU) care? Your input about your institution is valuable to help us improve LTFU care for Childhood, Adolescent, and Young Adult (CAYA) cancer survivors within Europe. Please fill out the ‘HCP’ survey. You can fill out the survey together as a team. If you fill out this survey by yourself then please share this survey with other health care professionals within your team.
