Young people who have had a rare and/or chronic disease in their childhood have to move from child-centred care to adult medicine at a certain point. This phase of transition is an important issue and different challenges have to be faced. Among them are gaps in care, health risks and declining quality of life for young people. Close cooperation at different organizational levels is necessary to overcome these challenges, to which the “Österreichische Liga für kinder- und Jugendgesundheit” aims to contribute.

For this purpose, a symposium will take place again this year on June 16 in Vienna, which will address the healthcare topic of transition.

Some of the agenda items are an overview of transition in Austria, Germany and Switzerland, the presentation of transition guidelines, as well as lectures on topics such as neuromuscular diseases, oncology, transition psychiatry, self-help and practical discussions.
